Jennifer Mabrey is the founder of Integrative Care Management LLC, which was established to provide private
care management services to those in need.
Jennifer is a graduate of Manatee Community College’s nursing program and has been a Registered Nurse since 2003. She has had a varied nursing career including working working in the hospital and/or home settings. Her passion for geriatric care directed her to continuing her healthcare career primarily focused on the aging population.
In 2020, Jennifer was nominated and appointed to the the board of directors of the Sarasota County Senior Advocacy Council . She is also an active member of the Aging Life Care Association and holds multiple specialty certifications including:
As a Care Manager, Jennifer’s passion is to provide comprehensive care management services in a private, gentle, and compassionate way. “I enjoy educating my clients and building long term relationships," Jennifer states. “My goal is to help my clients maintain independence and age in place for as long as safely possible.”
As a care manager I have a collaborative role in assessment, planning, facilitation and advocacy for options and services to meet individual health needs. I follow basic ethical tenants, including recognition of each individual and their right to self-determination, confidentially while considering their underlying mental status.
While my primary care focus is on the geriatric client, I have experience working with clients that have had traumatic or acquired brain injuries and mental health disorders. I am able to administer a variety of standardized tests to individuals to assess their mental and cognitive status. My many years of experience as an RN in this field allows me to successfully interface with the client, family, attorneys, physicians, insurance companies and various members of the health care team on a routine and necessary basis.
In many cases, frequent interaction with various insurance providers may be required, including but not limited to: Medicare, Medicaid, Long Term Care, various supplemental and replacement policies as well as auto no fault. I am frequently required to investigate denials with above insurance policies as well as facilitate appeals.

An Aging Life Care Professional, also known as a geriatric care manager, is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. The Aging Life Care Professional is educated and experienced in any of several fields related to aging life care / care management, including, but not limited to gerontology, nursing, social work, psychology, or occupational therapy, with a specialized focus on issues related to aging and elder care.
The Aging Life Care Professional assists clients in attaining their maximum functional potential. The individual’s independence is encouraged, while safety and security concerns are also addressed. Aging Life Care Professionals are able to address a broad range of issues related to the well-being of their client. They also have extensive knowledge about the costs, quality, and availability of resources in their communities.
Aging Life Care Professionals become the “coach” and families or clients the “team captain.”
Aging Life Care Professionals are members of the Aging Life Care Association® (ALCA) and differ from Patient Advocates, Senior Advisors, Senior Navigators, and Elder Advocates. ALCA members must meet stringent education, experience, and certification requirements of the organization, and all members are required to adhere to a strict code of ethics and standards of practice.
