What is a Care Manager?
Care Managers are healthcare providers that assist clients by matching their needs with appropriate services. Care Managers oversee client care at home, clinics, hospitals, nursing homes, and other health care facilities. Care managers assist families find better and more efficient ways of providing care for a loved one.
What is a Care Manager's role?
- Assessment of all needs, including by not limited to social, medical, personal, physical and cognitive, to prepare and implement individualized care plans
- Client advocacy
- Placement/relocation assistance
- Crisis management/intervention
- Familiar with available community resources
- Liaison between, client, family, financial advisors, physicians, specialists, authorized representatives, etc.
- Provide guidance or assistance to the fiduciary as it pertains to: management of assets, liquidation of personal property, sustainable planning
What is a Geriatric Care Manager?
A Geriatric Care Manager plans and coordinates care exclusively to the elderly to improve their quality of life and to maintain their independence. The geriatric care manager may have completed extensive training in any of several fields including nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care.
Who needs a Care Manager?

What is the cost of hiring a Care Manager?
It's important to note that in many cases insurance plans do not cover the cost of a Care Manager and Medicare does not pay for their services. Many times, the cost is paid by the family. Long-term care insurance may also cover the cost of a care assessment and a Care Manager.
