Care Managers are healthcare providers that assist clients by matching their needs with appropriate services. Care Managers oversee client care at home, clinics, hospitals, nursing homes, and other health care facilities. Care managers assist families find better and more efficient ways of providing care for a loved one.
A Geriatric Care Manager plans and coordinates care exclusively to the elderly to improve their quality of life and to maintain their independence. The geriatric care manager may have completed extensive training in any of several fields including nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care.

It's important to note that in many cases insurance plans do not cover the cost of a Care Manager and Medicare does not pay for their services. Many times, the cost is paid by the family. Long-term care insurance may also cover the cost of a care assessment and a Care Manager.
